COVID-19 Pandemic Disruption of Sixth Grade School Forms
Posted by Kristin Hayes on 11/18/2020The 2020-2021 school year has started out with many challenges for everyone. With remote learning being an option for families to choose, students and their families have not been able to turn in the required medical forms established by the state. Sixth grade requires a curent physical along with the immunizations to meet the state requirement. Tdap and Meningococcal are two of the required immunizations. Some children also need to have a second varicella and start the Hepatitis B series of three immunizations. Hepatitis B is given with the second dose given at least one month after the first dose. The third dose is given at least four months after the first dose and two months after the second dose.
We all hope to be able to return to in person learning in the New Year. When the time is established, I wanted to remind you that your child in sixth grade would be required to have their physical and immunizations completed and submitted to the health office on the first day the students return to the classroom.
The Illinois Board of Edcation states that a student that does not have all required documents submitted to the health office would not be allowed to return to the classroom. Students are not allowed on school grounds before or after the school day has completed. If you have, any questions please feel free to contact the health office.

6th Grade Dental Exams
Posted by Kristin Hayes on 8/1/2020The State of Illinois requires all 6th grade students to have a dental exam on file with the school health office. The dental exam is due by May 15, of the curent school year. If you have not already submitted a dental exam form this year, please download a copy from the medical forms link. The exam must not exceed 18 months from the May 15th date. If you have been to the dentist recently, please ask your dentist to fill out the dental form and turn it into the health office, or fax a copy to Lincoln Hall at 847-675-8124.
