After registering, please upload the following documents, email them to Jessica Tarnoff at jtarnoff@sd74.org, or bring them in to the Todd Hall Main Office.

REQUIRED HEALTH FORMS

1. Certificate of Child Health Examination: You will need a doctor’s physical and up-to-date immunizations. A lead screening or blood test and a diabetes screening must be indicated on this form.
2. Childhood Lead Risk Assessment Questionnaire: If you respond “yes” or “don’t know” on this form, your child’s doctor must perform a blood lead test.
3. Proof of School Dental Examination Form: Please have the dentist complete this form.
4. Eye Examination Report: Please have an optometrist or ophthalmologist complete this form.

RESIDENCY REQUIREMENTS

HOMEOWNERS:

• Property tax bill with proof of payment or closing statement
• Driver’s license or state ID showing Lincolnwood address
• One utility bill (gas, electric, or water)
• One of the following:
  • Voter’s card
  • Homeowner insurance certificate
  • Illinois vehicle registration

RENTERS:

• Valid original lease with landlord's phone number
• Proof of last two months' payments
• Driver’s license or state ID showing Lincolnwood address
• One utility bill (gas, electric, or water)
• One of the following:
  • Voter’s card
  • Homeowner insurance certificate
  • Illinois vehicle registration

LIVING WITH OTHERS:

FROM THE HOMEOWNERS:

• Driver’s license or state ID showing Lincolnwood address
• Two of the following from homeowners:
  • Voter’s card
  • Homeowner insurance
  • Vehicle registration
  • One utility bill (gas, electric, or water)
  • Owner Affidavit 2 of the registration packet

FROM THE PARENTS/GUARDIANS:

• Driver’s license or state ID showing Lincolnwood address
• Two of the following from parents/guardians:
  • Voter’s card
  • Homeowner insurance
  • Vehicle registration
  • One utility bill (gas, electric, or water)
  • Parent Affidavit 1 of the registration packet
