When Should Students Stay Home Sick?

Students should not attend school if any of the following conditions are present:
  • Temperature of 99.7 degrees or higher
  • Vomiting
  • Diarrhea
  • Incessant or deep cough, and/or purulent discharge from the nose
  • Red eye(s) with discharge and/or crusting on lashes and pain or irritation
  • Any skin lesions that are open and weeping or an unexplained rash
  • Contagious illnesses
  • Obvious signs that your child cannot comfortably participate in class activities (even without a fever)
Symptoms should be absent before the student returns to school. Children with communicable diseases will be excluded from school. Please contact your physician in these cases, and notify the nurse or specialist when a diagnosis is established.

Common contagious illnesses for which you should call the health office include:
  • Strep throat
  • Conjunctivitis (pink eye)
  • Head lice
  • Chicken pox
  • Impetigo

Lincolnwood SD74 operates under Illinois State Law and the Lincolnwood Board of Education policy. Depending on the grade a student is entering, physical examinations and proof of adequate immunizations are required.

  • Physical examinations cannot be more than 12 months in advance of the first day of school. All required medical forms should be completed and submitted before the first day of the school year.
  • Any child not in compliance with health requirements by October 15 of the current school year will be excluded from school that day until proof of compliance is presented.
  • All new students who are first-time registrants, entering after October 15, shall have 30 days following registration to comply with the health examination and immunization regulations.

Medical Forms

Please review the grade-level requirements below and submit all required forms.

Immunizations

The following immunizations are required for all students entering school.

The following vaccinations are optional, but recommended.

Vision and Hearing Screenings

Vision screenings will begin in the month of October for students in the following grades: Pre-Kindergarten, second grade, and eighth grade. Transfer students, students with special needs, and students referred by teachers will also be screened.

Hearing screenings will also be held for Pre-Kindergarten, kindergarten, first grade, second grade, and third grade students. Transfer students, students with special needs, and students referred by teachers will also be screened.

A vision screening is not a substitute for a complete eye and vision evaluation by an eye doctor. Your child is not required to undergo this vision screening if an optometrist or ophthalmologist has completed and signed an eye examination report indicating that an eye examination has been administered within the previous 12 months. A copy of the eye examination must be on file in the respective health office.
